Your initial psychiatric evaluation was our starting point. Your ongoing medication management follow-ups are where our partnership comes to life.

These sessions are far more than just a "refill." They are active, collaborative check-ins to ensure your treatment plan is moving you closer to your goals. As a Psychiatric Mental Health Nurse Practitioner (PMHNP), my commitment is to listen first, review what’s working (and what isn't), and adjust our course together.


Our Philosophy: Goal-Focused, Collaborative Care
Our main measure of success is your "north star"—the personal, tangible goals we set together. Are you sleeping more soundly? Are you navigating social events with less anxiety?

While we will always cover the clinical essentials like your symptoms and any medication side effects, we connect this data back to your real-world quality of life.

The primary focus of these appointments is your medication and symptom response. However, if you are facing an acute crisis or a significant new stressor, we can use a portion of our time for brief, supportive problem-solving to help you get through it.
How to Prepare for Your Follow-Up
To make the most of our focused session, it's helpful to quickly reflect on:

Symptom & Goal Progress: What’s better? What’s worse? How close do you feel to your goals?

Medication Experience: Note any benefits or side effects (even small ones). Have you missed any doses?

Key Life Events: Any new stressors, big changes, or major wins?

Your Questions: Write down anything you want to ask.​​


Refining Your Treatment Plan Together
At the end of our session, we will collaboratively decide on the next steps. This isn't a top-down directive; it's a shared decision.

Medication Decisions: We have three main paths:

Continue: If the plan is working well.

Optimize: If we need to adjust the dosage for better results or fewer side effects.

Pivot: If the current plan isn't a good fit, we'll discuss other options.

Holistic View: We will also reaffirm the non-medication parts of your plan, such as lifestyle changes, coping skills, or referrals to specialized therapists.

Scheduling: We’ll schedule our next check-in, often sooner if we're making a change, and further out if you're stable and doing well.
